Who says to school says new goals? As of this Monday, September 1, 500 general professionals are subject to objectives (MSO) decided by health insurance. It is a kind of withdrawal from the order aimed at professionals who would prevail too many labor births. “They prescribe at least twice the labor births compared to their concentrates with identical characteristics,” said the National Health Insurance Fund (CNAM) with BFMTV.
Specifically, doctors subject to an MSO must, for six months, to be able to prescribe less job births. “The objective of reduction, established by the Head of Service of the Doctor Council, depends on the level of prescription of doctors. It is between -20 and -30 %,” said the CNAM.
Rogneous doctors
If the MSO procedure has existed for almost 20 years, the CNAM approach is poorly received by doctors. “The profession experiences as an injustice because we get out of care, we return to the administrative values that have no meaning and do not respect the judgment of a health professional,” deplored Olivier Leroy, general doctor in Angers and a member of the coordination of free and independent doctors of Maine-Et-Loire.
Doctors also doubt, and above all, the method used by health insurance to go to a priori practitioners too generous in terms of labor strikes. She “takes data, of which we do not know exactly all sources, for us, they are quite wrong and judge negatively to health professionals,” said Olivier Leroy.
For its part, the CNAM indicates to BFMTV that the practices of doctors who are also prescribing for work strikes are attacked by an MSO after a study of their practices “compared to their colleagues in the same region, in patients and comparable exercise territory.
Another 500 general professionals put at the beginning in early 2026
The 500 doctors attacked on September 1 by an MSO are just a first wave of reminders of the order decided by health insurance. But “an equivalent magnitude is planned at this stage for the second wave that will begin on January 1, 2026,” said the CNAM in BFMTV. However, the latter can refuse to submit to him motivating his decision.
But then they can be put under prior agreement (MSAP). This procedure is even more restrictive than the MSO because the prescribed labor strikes are subject to the previous agreement of the Medical Control Service of the Primary Health Insurance Fund attached to the professional to order.
According to an evaluation established by the CNAM, “more than 400 mso and 200 msap were established between 2023 and early 2024”. And three quarters of doctors who have been the subject of an MSO have achieved their goals.
Daily assignment expenses
If the CNM attacks general professionals, it is because according to it, a part of the increase in daily assignments paid to disease licenses cannot be explained only by the aging of the population and the development of chronic diseases in the population.
In its “positions and products 2026”, a vast document published at the beginning of the summer to guide the public authorities of the measures that will be integrated into the next security budget, the CNAM indicates that “the daily assignments have increased considerably, with an increase of 27.9 % between 2019 and 2023”. And that, “if this increase is explained to 60% per demographic (increase and aging of the salaried population) and economic (average salary or minimum wage), the remaining participation is linked to an increase in judgments and their frequency,” he adds.
